REQUIRED DOCUMENTS:

• A copy of TWO proofs of address (examples: a bill, your state ID)

• A copy of your child's birth certificate or passport

• If available, a copy of your child's immunization record

• Admissions/Discharge/Transfer Form

• Child and Adolescent Health Examination Form

• Cumulative Record (only fill out the top part of the first page)

• Emergency Contact Information, Walking Field Trip Permission, Dismissal and Afterschool Routines, Health Information 2018-2019

• Federal Parent/Guardian Student Ethnic & Race Identification

• Photo/Video Consent Form

• Residency Questionnaire

• Parent Volunteer Form

Does your child have an IEP (INDIVIDUALIZED EDUCATION PROGRAM)? If yes, please bring:

• IEP (If applicable or in process)

• 504 (If applicable or in process)

For safety reasons, to enter the building, all parents, guardians, or anyone that may be picking up your child are required to have a school issued ID card. Your child does NOT need a photo ID card. To receive your ID card, please email IDCARD@PS333.ORG with the subject "ID FOR 2018-2019 (Include Grade)" with the following information: your name, your photo (selfie), your child's name.
